So despite using various moisturizers and foundations and also exfoliating more than once a week , my skin, especially my cheeks and forehead, look patchy under foundation. I've had this problem since I first started wearing foundation at 12. I personally like wearing foundation, but I long for a smooth finish. Also it looks patchy and dry without powder so that can't be a cause.
Any help or advice is welcome.

Do you use a primer? Have you tried a hydrating foundation like Laura Mercier?

Make sure you're using a primer, and keep in mind that different types of foundations and application can yield different results.
You can also use water to dilute it ( spray some on )

Chances are you have dry skin then. Try exfoliating once a week to once every two weeks. If you exfoliate too often you're actually irritating your already dry skin and causing it to flake up more, then I suggest using an over night problem night AND day, as it is twice as nourishing and hydrating as normal moisturizers. I definitely recommend Animo Genesis anti age moisturizer, I saw results on my dry skin after one day! Make sure your foundation is meant for dry skin, as they tend to be more hydrating. Garnier BB cream for dry skin is amazing as it's oil based and moisturizes you as you put it on.
I definitely suggest trying a 100% Mineral powder foundation though. Mineral makeup is never 100% matte so minerals foundation, though almost matte, still has a bit of a healthy glow/sheen to it and will help hide your dryness, it also doesn't need setting powder which may make your skin appear more dry than it is by mattefying it. I definitely suggest Micabella foundation, I've been using it for years, it's high coverage and a little goes a long way!

I have terribly dry skin and I use CeraVe Moisturizing Lotion and it has really helped my skin, they also have creams that are thicker if your skin is still dry. I use Benefit Professionals and I love it, and for foundation I have been using Clinique Even Better and it has been the best foundation for my super dry skin. I would go easy on the exfoliation because that could be making it worse. I also have been using CeraVe Hydrating Cleanser and it works really well. I also tend to have cakey looking foundation depending on the way I apply foundation, so that could be your problem. I have tried countless brushes and so far Sephora's Pro Airbrush (not exactly sure on the name but I will post pic) has been really good for applying my makeup

And I also recommend a mineral foundation like Brittany mentioned. I use merle norman mineral foundation and I love it. I apply with elf's Kabuki Brush and I still use primer with it. And if I have blemishes I cover with hard candy Glamoflauge and then apply the powder. BareMinerals is really good too and L'Oreal makes one similar to Merle Norman (look at their website, you have to buy products in their store)

I use to use the same brush but I have gotten a better finish with the Sephora brush, so glad I switched:)
