A wash with Salicylic acid (either made specifically for your body or one for your face and body, like Cetaphil or Dermalogica's Clean Start cleanser). Use a loofah the first few times because the skin on your back is much tougher and needs exfoliating. Try to wear tank tops when exercising and showering right after you've sweated a lot so it doesn't clog your pores. Also, before you get out of the shower, make sure that there's no shampoo residue left on your back.

I went to the dr for cystic acne (large sore red zits that never get a head and last weeks) and he gave me tea tree oil pads. cleared up within a month and never came back!
