When I use a base coat and top coat on my nails with nail polish it starts to peel off within 12 hours! What can I do?

Probably the brand of nail polish you are using. Have you tried higher end brands like OPI?

Your nails probably had too much oil or moisturizer etc soaked into them when you painted. Wash your hands and wipe a bit of remover on the naked nail before painting. If that doesn't work out, I recommend trying a new base coat. Doesn't have to be higher end, just check out the reviews here. :] My favorite base coat is only $4!~
