Paint the bottom half of your nail a with a red colour and paint the other half white. Then draw a black line down the middle; you could use a fine brush or a black nail-art pen. Lastly, you can use a medium size dotting tool to make a black dot in the middle of the black line you created earlier and then a small size dotting tool to make a white dot on top of the black. However, if you do not own any dotting tools, a q-tip will also work but you will need to flatten the tip when making the black dot, otherwise the black and white will be of equal shape and there won't be an outline of the black.
