I don't think you should lose all your piercings, just wear small appropriate earrings :)

I don't think having piercings will make you and less of a med student so I don't think having piercings in your ear will define you as being trashy. :)

I know how it feels girl, I have a LOT of piercings and I get what it feels like to have people assume the worst just because you have them haha, your ear piercings are nice :) don't listen to the people who tell you otherwise 😘

I like all of them, but if you'd feel more comfortable getting rid of one, I like 2 and 3. Number 1 looks a little weird. Lol. And unless your instructor/boss/etc. has specifically told you that you need to remove them, I wouldnt worry about it! Wear appropriate jewelry when youll be exposed. Criticism is hard to take, but if you like them let peoples opinions roll off your back. I have my nose and septum pierced, and 4 tattoos, and my family has something to say nearly everytime I see them. They make me happy and confident though, so I push on. :)

2 & 3 :) If I had a doctor/nurse with piercings (I probably have but haven't noticed lol) I wouldn't think they were trashy at all. I would probably be more comfortable with them to be honest!

I think ear piercings won't be a huge "image" problem for you (as long as they aren't stretched). Most people don't find ear piercings too rebellious or out of the norm nowadays, particularly when people are putting so much metal everywhere else. ;) If you are worried, you can wear a clear retainer and just wear jewelry in the holes you find comfortable.
As for healthcare workers with tattoos: a lot of older people don't trust them, because they are stuck in the outdated mindset that criminals and rowdy sailors get tattoos. If you get one, I suggest thinking carefully about size and placement, so that you can cover it easily if you choose. Also, people who don't like tattoos at least seem to appreciate if it is well done, and has personal significance to you rather than just a random picture.
