Make sure you apply the foundation well all over the face and neck,make sure it also matches your skin. Maybe you need a new one,make sure your face is fully cleansed before applying so there is no dirt left on the skin when applying.After applying make sure you set your makeup with a powder,otherwise your makeup will look patchy.

Uhm... Maybe you need to change your primer... I know something similar happened to me and I changed my primer and it got better. I'd say to also reconsider what foundation your using. Also look into getting a good toner. Try to apply your foundation with with a brush instead of fingers until the end of your application. Uhm... That's all I can think of...
