It is a triple forward helix piercing. Most people don't have the proper shape for that piercing to work; the majority of people either can't fit multiple studs or the backs show under the helix cartilage.
The best thing to do if you are interested in this is to talk to a reputable piercer in your area (meaning one who is trained, uses proper technique with a hollow-point needle, and practices thorough sanitation). The consultation should tell you if this is possible on your ear, how much it would cost, and how long it would take (you shouldn't pierce all three at once, as the swelling is too great and will cause the cartilage to collapse or the piercings to get infected). Be forewarned that this is not cheap; a good piercer charges <$50 per helix piercing, plus jewelry.

(I had 4 cartilage piercing done all at once and my cartilage didn't collapse or get infected. I also don't pay extra for jewerly where I'm from. Just a personnel experience.)

I have one piercing done on each side but not the 3 cause they hurt so bad when getting pierced haha worth getting them though :)
