Dermatologist. Or try Clarisonic. It was by far the best thing I ever did (even going to a derm and different times or different dr didn't help). It made a huge improvement on my skin but you have to use it right (not pressing too deeply into the skin), consistently (I use mine every night and I remove my makeup before), and you have to have the right brush for your skin type (this was the hardest part for me because I wasn't sure which one was the best for me). I also use my own face washes and not the Clarisonic ones. The most important thing though is to never ever everrrr skip washing your face even if you don't have a clarisonic. I still washed my face right after I had surgery because my skin gets horrible if I skip. Not wearing foundation was also helpful. I started only wearing concealer where need (primer before) and loose powder like Merle Norman Mineral Powder, Bare Minerals, or Tarte has a good powder too. I also recommend using a toner and alwaysssssss moisturize! I recommend CeraVe Moisturizing Lotion and Murad for cleansers and toner. Also Philosophy Clear Days is great too.

Talk to your doctor about it - it does not have to be a dermatologist. In my experiences, my GP and Derm have always given me identical advice. If you don't get results from the treatment your doctor gave you, and you're running out of options, only then should you consider seeing a derm.
I highly recommend that you read this article: bit.ly/18tXfNi
Usually when you have acne that refuses to go away, it's a sign of hormonal acne, which will not go away until your hormones are in balance again. No amount of skincare products or facials will clear your skin. Your dr. will most probably put you on acne medication, possibly along with a topical gel. The medication will clear your acne while the topical gel prevents it from returning. If that doesn't work - which is rarely the case, but it does happen to a few of us unfortunate ones - your doctor will probably start talking to you about more 'drastic' options like birth control and Accutane.
I hope this helped. Best of luck! I've been there, done that, and I know how much it sucks, so let me know if you have any other questions.
