Use a primer then a brush to apply the concealer instead of smearing it on. It'll cover better too if you use a brush. If that doesn't work it could just be the formula, and it's drying on the blemish that's why it looks cakey.

Maybe mix it with a little bit of water I don't know if it will work I don't have that concealer.

Applying eye cream to the blemish should help with the patchiness.
