Most all, but it could be in the way it's being applied. I would say you should try getting a full coverage foundation, since your concealers should be lighter than your foundation. You don't want to have light spots all over your face from concealing acne spots. I use a concealer from the Sephora Collection and that seems to work for me. But let a full coverage foundation do all the work first and then go back and cover up breakouts lightly with a concealer, second. Buff it on your face in thin layers to get maximum coverage. But beware, that much foundation can leave your makeup looking cakey, so after you're done, spray your face with a finishing spray to give your skin a hydrated look.

Kat von d foundation is great, usually don't need concealer (can't recommend her concealer because none of them match me.. urban decay naked foundation is also pretty good but still need a little concealer at times.
