I usually put the crease colour I want onto a small angled eyeliner brush and then create a line in my fold.... lines... yeah... ahahaha. I did a video with a cut-ish crease so it's easier to show than say - http://www.youtube.com/watch?v=8k12qTCvcco

I apply the lid colour then take a pencil liner and map out where i want to put the darker colour then when im happy with it i use a small pencil brush and put a darker colour overtop of the pencil :)

I prime first, then take a pencil brush and apply a matte contouring shade along the browbone (just above the actual crease). I usually use MAC Cork on myself, I would probably use something like MAC Handwritten on you. Then take a skin toned shadow or your regular face powder and cover the lid up to the crease. Blend out that harsh crease line using small circular motions. I should probably add that it should look pretty weird until you blend it out!
